2. Submit Information:
   Send an email to leeshopservice@outlook.com with the following information, answering the questions from Question A to Question I. We will upload this information to an online hospital platform to help the doctor better understand your situation.

   • Question A: What is your name?
   • Question B: What is your gender?
   • Question C: What is your date of birth?
   • Question D: What are your height and weight?
   • Question E: How long have you been unwell?
   • Question F: Do you have any allergies?
   • Question G: What is your medical history?
   • Question H: Description of your current illness?
   • Question I: Do you have any previous medical reports, such as an Ultrasound or X-ray report? If so, please send them to us.
   • Question J: What medications are you currently taking?

3. Provide Specific Questions:
   List any specific questions you would like the doctor to answer.

4. Translation and Response:
   We will translate your questions into Chinese and forward them to the doctor. Once we receive the doctor's responses, we will translate them back into English and send them to you. This process typically takes up to 48 hours.

5. Refund Policy:
   Once the service has commenced, refunds will not be available. However, we will do our utmost to ensure your satisfaction.
