Indications: Gangrene caused by excessive heat and toxins. Symptoms include dark red, slightly swollen, and hot limbs, severe ulceration with foul odor, intense pain, fever, thirst, red tongue, and rapid pulse.
Formula Rationale: This condition is often caused by damp-heat toxins, which transform into heat and obstruct the blood circulation, leading to muscle decay. The treatment focuses on clearing heat and detoxifying, promoting blood circulation, and relieving pain. Honeysuckle, being cold and sweet, enters the heart and is effective in clearing heat and detoxifying, thus it is used in a large amount as the main ingredient. Angelica promotes blood circulation and disperses stasis, Figwort clears fire and detoxifies, and Licorice clears various toxins and enhances the heat-clearing and detoxifying effects of Honeysuckle. The combination of these four herbs not only clears heat and detoxifies but also promotes blood circulation and disperses stasis, making it an excellent formula for treating gangrene.
Application: This formula is used for gangrene caused by excessive heat and toxins. The clinical application focuses on symptoms such as dark red, slightly swollen, and hot limbs, severe ulceration with foul odor, intense pain, fever, thirst, red tongue, and rapid pulse.
Modifications:
For severe damp-heat: add Phellodendron (Chuanbai), Atractylodes (Cangzhu), Anemarrhena (Zhimu), and Alisma (Zexie).
For obvious blood stasis: add Peach Kernel (Taoren), Safflower (Honghua), and Japanese Knotweed (Huzhang).
For deficiency of both qi and blood: add Codonopsis (Dangshen), Astragalus (Huangqi), Rehmannia (Shengdi), Atractylodes (Baizhu), and Spatholobus (Jixueteng).
