Instructions for Decocting and Taking Traditional Chinese Medicine (TCM) Decoctions

  1. Suitable containers for decocting medicine include earthenware pots, enamel pots, and stainless steel pots. Do not use iron or aluminum pots.
  2. Before decocting, soak the herbs in cold water for 20-30 minutes. Generally, add enough water to cover the herbs by 3-5 centimeters.
  3. For general herbs, bring to a boil over high heat, then reduce to a gentle simmer and cook for another 20-30 minutes. Diaphoretic (sweat-inducing) herbs are generally simmered for 10-15 minutes after boiling, while tonic herbs are generally simmered for 30 minutes after boiling. Each dose of herbal medicine is typically decocted twice. After pouring out the liquid from the first decoction, add an appropriate amount of water for the second decoction; the boiling time can be slightly shortened. The liquid from the first and second decoctions can be taken separately in two doses, or the liquids from both decoctions can be combined and then divided into two doses. If there are doctor’s orders regarding the time of administration, follow them; if not, it is generally taken 30 minutes after a meal.
  4. Decoction volume: The liquid volume per dose for adults is 150-200 milliliters.
Special decoction methods: For special herbs requiring “decoct first” or “add later,” follow the doctor’s instructions regarding the timing. If there are no doctor’s instructions, operate according to the following methods:
① Decoction first (先煎): Boil the specific herb for 10-30 minutes before adding the other herbs to decoct together.
② Add later (后下): Add the specific herb to the decoction 5-10 minutes before the other herbs are fully cooked.
③ Decoct in a bag (包煎): Some herbs easily irritate the throat or stick to the pot. They need to be wrapped in a gauze or peach paper bag for decocting; do not remove them from the bag without authorization.
④ Melt or dissolve (烊化、冲服): Add the herbs that need to be melted or dissolved into the finished, strained decoction. Stir continuously until completely melted, and then it is ready to be taken.
